(For Members Residing in Thunder Bay Ages 16 & 17 Only)
ANY 16-17 year old KZA-GBFN Members residing in Thunder Bay who wish to receive their Pfizer Vaccine please fill out the form on gullbayfirstnation.ca website.
Gull Bay is currently preparing a list of any Thunder Bay Residing Members who did not meet the age criteria of 18 years and older for the Moderna Vaccine who would like still like to receive the Pfizer Vaccine which has been approved for people ages 16 and up.
If you are not 16 or 17 years old please do not fill out this form. Future vaccine clinics will take place for other members.
Vaccinations will be taking place on a yet to be determined date between April 26-30th, 2021.
